Alumni may order a replacement diploma by completing and mailing the following .

On the form you will provide the following:

• Your name as you attended 61传媒and current legal name
• Year of graduation
• Birthdate
• Provide your email address and phone number
• Address to where you wish to have the diploma sent when ready

Mail the completed form and payment to:

The fee is $75.00, so make a check or money order payable in this amount to 61传媒 and include with your completed form. Additional fees may incur for shipping international or via FedEx.

Replacement orders may take between 6-8 weeks for processing.
